My only remaining idea is to delete the whole Documents/My Games/Freelancer folder. Maybe the files got corrupted in some way. Deleting them lets Freelancer re-create them.
OR
I can open the keymap.ini and edit the keys, see if the anti-cheat kicks me or the controls stick and don't reset.

I like how you're thinking!
You can edit the keybinds file, even set it as read-only if the game still refuses to cooperate.
The file references keys as numbers, so you'll need to use a table like this.
I've done that before, so let me know if you need help doing that.
Also if you have a second working PC, you can get the keybinds file from there (again set it as read-only if needed).
Update:
Modifying the UserKeyMap file does nothing - upon closer inspection and cross-validation of the keys in the file with the key codes in the link you provided, the UserKeyMap file in Documents\MyGames\Freelancer is up to date with the modified controls (these controls are setup 1 year ago and match the controls keys I wanted and assigned).
Upon deletion of UserKeyMap file, and game re-launch (launcher included), the file was not created again.
On verification of DSAce, last entry to it is 31.05.2024. Since then it has not updated.
BUT:
Modifying the keymap Configuration file in K:\Discovery Freelancer 4.91.0\DATA\INTERFACE binds all the controls as I want and does not trigger the Anti-Cheat or any other issue.
And exiting the game keeps the controls as I set them.
I am inclined to say that this issue is not related to anything on my OS / PC.
Friend of mine got the same problem. Tried all the suggested solutions, but nothing worked so far. The Keymap file in the discovery folder look very complicated and we're scared to change anything there.
